What You'll Do:                                                 

The Specialty Telemedicine Physician will work directly with patients together with a team of Cardiovascular Nurses, Respiratory Therapists, Registered Dietitians, and other clinicians. The physician will be responsible for overseeing patient care by providing direct video/audio consultations and supervising the care provided by the non-physician clinical team. This is a remote, part-time, and independent contractor position.

Primary Responsibilities:

  • Provide video/audio visits with patients requiring expert intervention in the management of their chronic diseases. The patient’s non-physician clinician may also attend these visits to better carry out the physician’s care plan.
  • Supervise patient care delivered by non-physician clinicians through case review meetings. The goal is to guide the clinicians in appropriate medication adjustments and titration.
  • Support patients on all medical therapies utilized in the management of their chronic diseases including cardiometabolic diseases, diabetes, chronic kidney disease, asthma, and COPD.
  • Other duties as assigned

Requirements and Experience: 

  • MD or DO degree required
  • Active MD (Doctor of Medicine) or DO (Doctor of Osteopathy) license in Minnesota and/or Wisconsin.
  • Board Certification
  • Active DEA license
  • Ability to work collaboratively within a clinical team
  • Strong computer skills
  • Expertise with supporting patients on diabetes therapies (orals, injectables, insulin, etc.)
  • Expertise with supporting patients on diabetes technologies (pumps, CGM, connected pens, apps, etc.)
  • Proficient with EMR platforms and medical billing
